Transfer Records

As a patient transferring to our new office, the most important information we need is as follows:

• Date of your most recent check-up x-rays
• Date of your most recent panoramic x-ray
• Date that your next check-up and cleaning is due

If we need a copy of your records, we can request that after your first visit.

While your previous records are sometimes helpful, they are not required.  If you have not received regular care in over a year, (or if you would rather not get your old records) we will simply evaluate your current condition and begin your treatment from there.  Either way you can be assured that we will treat you like family!
